Voluntary Vision Insurance

All contract employees and all non-contract employees working 30 hours or more per week are eligible for vision insurance coverage. New employees must complete online enrollment within 31 days of their eligibility date to be covered (if eligible for benefits on May 1, enrollment must be completed by May 31). Please contact Human Resources to confirm your eligibility for benefits — most employees are eligible for benefits the first of the month after date of hire. Premiums are paid through payroll deduction and may be sheltered from income tax under Section 125.

Instructions for enrolling online.

The Vision Insurance Plan is offered through Superior Vision. By enrolling in the Superior Vision plan, you have access to comprehensive vision coverage through a Superior Vision provider:

• Exams are covered in full after a $15 Copay (every 12 months)
• Glasses, including frames up to $125, are covered in full after a $25 Copay (every 12 months)
• If you choose contacts over glasses, Superior Vision will pay for up to $130 in contact lenses
• Discounts, ranging from 10 – 30% on Laser Eye Surgery, prescription sunglasses and lens extra

This plan is voluntary, so you will pay the entire cost for coverage in Superior Vision.
