Items needed for enrollment:
- Proof of adress — purchase agreement, utility bill (gas or electrict — phone bill is unacceptable), bonafide rental agreement (that includes utilities)
- Birth certificate or I-94
- Immunization records
- If foster care, EEIF (red notebook) and homeless affidavit
- It is helpful to have transcript
