Dates still available to register:
- Thursday, April 7th — Washington High School (2:00pm - 7:00pm)
Click on the REGISTRATION tab at the left to schedule your screening.
You may have your doctor complete the health screening form as an option to attending a district Health Screening. Either option will require you to complete the Health Risk Assessment online at www.bluekc.com to receive get the $300 prescription deductible waived, effective July 1, 2011.
Deadline for turning in Wellness Verification form to Kate Foster at Central Office AND completing the online Health Risk Assessment is May 6, 2011.
